COVID-19 TESTING – SUITE 105

DO NOT GO TO THE CLINIC.  TESTING is done at SUITE 105, next to the clinic. When you arrive at the parking, please text the phone number 972-482-7400 to let us know you have arrived. We will remotely register you from your car and direct you into the clinic when it is your turn to be seen.

Please choose the reason for your visit and provide your information below. We will then reserve your place in line until your estimated visit time.
